52.50 per hour SUMMARY OVERVIEW

The AFC Registered Nurse functions as an integral part of an interdisciplinary case management team and participates in risk assessment and service planning for new and ongoing clients. The Registered Nurse (RN) functions as a health care provider, educator, consultant, and nurse case manager in the support of client care. The nurse troubleshoots problems and crises, anticipates complications and helps to prevent their development; and views the client and family within the context of a whole system. ESSENTIAL FUNCTIONS

The Registered Nurse works with Program Manager, RN Clinical Nurse, and Case Managers, to carry out the day-to-day responsibilities of managing Adult Family Care members. Conducts on site assessments, assesses the mental, physical and medical needs of clients, performs risk assessments, and ensures that identified needs are encompassed in the overall client care plan. Develops or implements individualized multidisciplinary care plans for new and ongoing BSHC Adult Family Care clients with the input of the Case Manager. Performs interdisciplinary reviews, and makes recommendations related to behavioral needs, medical needs and special programs that are regarded as required elements of the individualized multidisciplinary care plan that must be completed by RN’s and case managers for Adult Family Care clients. Participates in the case management peer review of client records and provides consultation, direction and feedback to Program Manager and case managers regarding mental health, behavioral issues, medical issues, nursing issues, insurance reimbursement and community resources. Ensures that medical and clinical care needs are identified and formally included in the case management plan for a client. Collaborates with Program Manager, case managers and nurses to promote cost effective service plans, and to determine need and eligibility for special programs funded by EOEA, Mass Health, and/or other community resources. Performs clinical assessments to screen potential adult foster care clients in the BSHC and outlining areas for programs and services funded by EOEA, Mass Health and other community resources. Participates in the evaluation of potential consumers and recommends appropriateness for special programs including Adult Family Care, Group Adult Family Care, ECOP and Choices. Provides technical assistance (theoretical and clinical expertise) and training in the screening process to hospitals, certified home health agencies, nursing facilities, ADH and PACE programs. Participates in and as needed develops and implements in service training programs, provide technical (theoretical and clinical expertise) on mental health, behavioral and medical issues for all level of agency staff. Maintains collaborative relationships with agencies and institutions that serve or interface with BSHC clients and or staff. Provides consultation to staff, management, administration and other disciplines related to his/her area of expertise. Provides input, direction and expertise in the development of agency programs related to their area of expertise. COMPETENCIES

Knowledge of Psychiatric, Mental Health and Geriatric Nursing. Knowledge of the home care system and community resources. Ability to communicate effectively and work well with others. High level of computer skills and knowledge. SUPERVISORY RESPONSIBILITY

This position does not have supervisory responsibilities. WORK ENVIRONMENT

This job operates in a professional office environment. This role routinely uses standard office equipment such as computers, phones, printers, copiers, filing cabinets. PHYSICAL DEMANDS

The employee must be able to carry 15 to 20 lbs. The employee is regularly required to talk and hear. The employee is frequently required to stand, walk, use hands to finger, handle or feel, and reach with hands and arms. POSITION TYPE/EXPECTED HOURS OF WORK

This is a per diem position without a standard schedule. Our operating hours are Monday-Friday, 9:00 AM -5:00 PM. TRAVEL

This position requires up to 25% travel. Must be able to travel to the community to perform visits at consumers' homes (may have limited accessibility), and travel under all weather conditions. REQUIRED/PREFERRED EDUCATION AND EXPERIENCE

Must have a valid Registered Nurse's license from the Commonwealth of Massachusetts. Must have a valid driver's license. Must have a Bachelor's degree in Nursing or an Associate of Science Degree in Nursing and at least one (1) year of clinical experience and/or nursing in a community or long term care setting; or an Associate Degree in Nursing and five years of experience, with two years of community based nursing or long term care setting. A Diploma RN and two (2) additional years of community-based nursing experience or long term care direct service experience. ADDITIONAL ELIGIBILITY QUALIFICATIONS
