Job Description Family Guidance Centers, Inc. (FGC), a not-for-profit behavioral healthcare organization, treats and prevents substance use disorder and related problems. Essential Duties

Conduct patient biopsychosocial assessments using standard practices.

Develop and maintain diversion relationships with each police district, serving as a liaison to connect individuals to treatment services offered by FGC.

Assess substance use severity.

Provide case management, discharge planning, and after-care coordination.

Develop and direct prospective referrals to FGC and other treatment providers, as appropriate and in accordance with clients’ needs and wishes.

Work closely with patients with substance use, mental health, or co-occurring challenges to enhance their recovery.

Provide individualized, ongoing guidance, coaching, and support while maintaining appropriate professional boundaries.

Work Schedule 40 hours per week (4:30 p.m. – 12:30 a.m.), including Saturday or Sunday.

Work Location 2630 South Wabash Ave., Chicago

Pay Rate $23.50 – $26.44 per hour

Preferred Start Date December 1, 2025

Required Experience Education: High School Diploma or equivalent. Must be at least 18 years old. Must have at least one year of recovery from substance use, mental health, or co-occurring challenges.

Certificates, Licenses, Registrations Certified by the Illinois Certification Board (ICB, Inc.) as a Certified Recovery Support Specialist (CRSS) or as a Certified Peer Recovery Specialist (CRPS), or be certified within one year of employment.
