Registered Nurse (RN) Patient Care Navigator - Outpatient Heart Failure
Location: Falls Church, VA. Full time, day shift. Must be flexible for rotating to Oakville & Mark Center locations in Alexandria, Woodbridge, and Loudoun. Sign‑on bonus and relocation assistance eligible.

Job Responsibilities
Coordinates a patient’s plan of care including appointment scheduling, attendance confirmation, transportation, referral process, and answers patient questions.
Acts as a liaison and advocate for patients needing screening, diagnostic treatment, clinical research participation, support services, and follow‑up plans by helping them access the best healthcare outcomes.
Refers cases to appropriate resources. Anticipates patient needs and advocates for patient rights and appropriate utilization of services.
Conducts assessment of the patient’s health and psychosocial needs and develops a case management plan in collaboration with the patient and their support system.
Educates patients, families, caregivers, and the health care delivery team regarding treatment options, community resources, insurance benefits, and self‑care management to facilitate timely and informed decision‑making.
Collaborates with multidisciplinary teams to address patient questions and responses about the care process and treatment options.
Develops and maintains reliable systems to document, track, and monitor patient data.
Performs other duties as assigned.
Minimum Qualifications
Education:
Bachelor’s Degree in Nursing.
Experience:
3 years of nursing experience.
Certification:
Licensed in the Commonwealth of Virginia as a Registered Nurse; Basic Life Support from the American Heart Association.
Preferred Qualifications
Experience:
Telemetry, ICU, CVICU, IMC step‑down; heart failure experience is a plus.
