Social Worker
to join our team in Brooklyn and help support residents and their families with care, guidance, and advocacy. What You’ll Do
Conduct resident and family interviews to develop detailed social histories and individualized care plans. Work closely with families to design inclusive programs that promote residents’ emotional, social, and psychological well-being. Provide guidance and solutions for challenges such as financial resources, discharge planning, and community support services. Collaborate with an interdisciplinary team—including administration, nursing, and medical staff—to ensure holistic and coordinated care. Advocate for residents by addressing their social and emotional needs while maintaining clear communication across the care team. Qualifications
Minimum of 2 years of experience as a Social Worker in a Skilled Nursing Facility (SNF) required. Strong interpersonal and communication skills, with the ability to build trust with residents, families, and colleagues.
