Crown Heights Center for Nursing and Rehabilitation is looking for a Social Worker / Geriatric Behavioral Health Specialist to join the team at our Skilled Nursing Facility in Brooklyn, NY.
The Allure Group is a consortium of six rehabilitation centers across Brooklyn and Manhattan, NY, specializing in post‑acute and sub‑acute skilled nursing and rehabilitation. We pride ourselves on providing a high standard of care for patients of all ages and conditions while upholding an environment of kindness, dignity, and respect for employees and residents. As a Social Worker at The Allure Group, you would be part of a winning team doing meaningful work to improve residents' quality of life, health, and comfort.
Responsibilities
Conduct behavioral health assessments for new and existing residents.
Develop and implement individualized behavioral care plans in collaboration with the interdisciplinary team.
Provide counseling, crisis intervention, and therapeutic support for residents experiencing anxiety, depression, dementia‑related behaviors, or other psychiatric issues.
Offer education and support to staff and families on behavior management techniques and mental health awareness.
Collaborate with medical, nursing, social work, and activities teams to ensure holistic resident care.
Monitor and document progress and adjust treatment plans as needed.
Participate in care planning meetings and regulatory surveys.
Requirements
Bachelor’s degree in Social Work, Psychology, Counseling, or a related field.
Knowledge of behavioral interventions for dementia and other age‑related conditions.
Strong clinical assessment and documentation skills.
Preferred Experience
Previous experience working with geriatric populations, especially in long‑term care or SNF settings.
