CardioRenal Vision (CRV) is a well-established, physician-led multispecialty group dedicated to providing exceptional cardiology and nephrology care for medically complex patients in skilled nursing facilities. We are currently seeking a motivated and compassionate
Nurse Practitioner (NP) or Physician Assistant (PA-C)
to join our team in the Tacoma, WA area. As a CRV specialist, you will practice with a high level of autonomy while benefiting from one-on-one mentorship and ongoing support from our supervising Cardiologists, Nephrologists, and support team. You will participate in a robust onboarding program designed to build confidence and ensure clinical excellence. Key Highlights

Prior experience in skilled nursing, internal medicine, family medicine, geriatrics,
cardiology , or
nephrology
is strongly preferred. Proven ability to collaborate with facility staff, including nursing directors, executive leadership, and other advanced practice providers. Commitment to ongoing clinical education and a proactive approach to patient care. Compassionate, patient-centered approach to managing chronic and acute conditions in a vulnerable population. Primary Responsibilities
Deliver high-quality Cardiology and Nephrology care to Skilled Nursing residents Collaborate with facility and primary care leadership Provide education and support to facility staff on relevant clinical topics Meet and maintain monthly encounter goals Serve as a clinical resource in cardiology and nephrology for the care team Complete documentation accurately and in a timely manner At CRV, we believe in empowering clinicians to grow, lead, and make an impact.
