HIRING A NURSE PRACTIONER OR PA

CardioRenal Vision (CRV) is a rapidly growing post-acute Cardiology, Nephrology, and Pulmonology established practice.

Currently, we are looking to fill an immediate full-time position for a qualified Nurse Practitioner (ARNP) to join this dynamic practice. The ARNP will work with sub-acute and long-term patients with supervising Physicians in Skilled Nursing Facilities (SNFs) in Tacoma and Puyallup, WA.

Job Type : Full-time (M-F) no weekends or call (flexible schedule)

About the Role The Advanced Practice Provider will serve as a vital healthcare provider delivering comprehensive specialty services with a focus on Cardiology and Nephrology. This role involves conducting thorough clinical assessments to diagnose and treat a wide range of acute and chronic conditions while prioritizing patient safety and quality care. The Advanced Practice Provider will collaborate closely with interdisciplinary teams to develop and implement individualized care plans that address complex health issues. By integrating evidence-based nursing practices, the role aims to improve patient outcomes and enhance overall healthcare experiences. Ultimately, the Advanced Practice Provider will contribute to advancing community health through compassionate, patient-centered care and continuous professional development.

Responsibilities

Provide direct patient care, including detailed clinical and physical assessments, treatment planning, and medication management.

Develop, implement and manage individualized treatment plans.

Monitor patient progress and adjust treatments as necessary to ensure optimal outcomes and patient safety.

Collaborate with physicians, nurses and other healthcare professionals to coordinate comprehensive care.

Educate patients and their families on health maintenance, disease prevention, and management of chronic conditions.

Utilize EMR systems to maintain accurate patient records and ensure continuity of care.

Collaborate with healthcare team members to deliver comprehensive patient care.

Stay updated on the latest medical advancements and best practices in nursing.

Demonstrate empathy, professionalism, and excellent communication skills in all interactions with patients and colleagues.

Skills The Advanced Practice Provider will utilize clinical assessment skills daily to accurately diagnose and treat patients, ensuring that care plans are tailored to individual needs. Knowledge of geriatrics allows the practitioner to address the unique health challenges of these populations effectively. Collaboration and communication skills are essential for working within interdisciplinary teams to coordinate care and improve patient outcomes.

Minimum Qualifications

Current and valid Advanced Practice Provider license in Washington.

Master’s or Doctoral degree in Nursing or Physician Assistant from an accredited program.

Demonstrated experience in clinical assessments and patient management.

Basic Life Support (BLS) and Advanced Cardiovascular Life Support (ACLS) certifications.

Preferred Qualifications

Experience working with geriatric and pediatric populations.

Experience in skilled nursing facilities.

Experience in a multidisciplinary healthcare setting.
