Position Summary HousingPlus is hiring a Social Worker to provide trauma-informed, evidence-based clinical and supportive services to women and families in a permanent supportive housing program. Participants include individuals and families impacted by serious mental illness, substance use disorders, justice reentry, physical disabilities, and other complex needs. The Social Worker works collaboratively with case managers and the broader support team to ensure comprehensive, coordinated care that promotes housing stability, wellness, and self-sufficiency.

Skilled in de-escalation, safety planning, and crisis response.

Responsibilities

Provide individual, family, and group interventions using evidence-based practices such as motivational interviewing, CBT, harm reduction, and trauma-informed care.

Conduct psychosocial assessments and develop individualized service and treatment plans.

Collaborate with case managers to identify and support participants with higher acuity needs.

Facilitate referrals to external providers for mental health, medical, legal, vocational, and community-based services.

Provide crisis intervention, de-escalation, and safety planning.

Maintain timely and accurate documentation in compliance with policies and funder requirements.

Participate in case conferences, staff meetings, and interdisciplinary care coordination.

Support program initiatives aimed at enhancing participant engagement, housing stability, and family well-being.

Provide in-home and in-office supports, including home visits as needed.

Provide clinical supervision or mentorship to social work interns as assigned.

Required Skills & Abilities

Minimum 2 years’ experience working with populations experiencing SMI, SUD, homelessness, reentry, or related challenges.

Knowledge of permanent supportive housing, harm reduction, and recovery-oriented practices preferred.

Experience working with families and women with complex needs is strongly desired.

Proficiency with electronic health record or case management systems.

Must be able to walk distances of .25 miles, walk over uneven ground, and climb a minimum of 3 flights of stairs.

Bilingual skills a plus.

Education & Experience

Master’s Degree in Social Work (MSW) required.

New York State LMSW or LCSW licensure strongly preferred.

Additional Schedule Details

Based in a congregate or scattered site permanent supportive housing program setting.

Combination of office-based work, participant home visits, and community outreach.

May require evening or weekend hours depending on participant needs.
