You, our Ideal Candidate This role will provide onsite support as the Enhanced Service offerings of Care Coordination and Home Services begin at Concord Reserve. This individual will be an LPN who will play an integral part of campus associate relationship building as resident needs are identified. This position will be responsible for maintaining contact with residents who are receiving care coordination as well as assisting to educate residents on Care Coordination and Home Services as appropriate. This individual will work as a team member and use their specialized credentials and/or expertise to serve clients. This position requires excellent communication skills with team members, clients and family members and an elevated level of patience and compassion for others.
Responsibilities
Manage care and services provided by Associates and ancillary staff working in Life Enriching Home Services. This management extends to any LEC (Life Enriching Communities) campus where Home Services are being delivered, and includes coordination and delivery of Associate onboarding, development, and delivery of Associate education to meet any applicable educational requirements, and ongoing evaluation of Associate performance.
Develops, coordinates, and implements service plan by using a collaborative approach to identify the client's wishes and abilities for optimum level of functioning and maintenance of dignity, independence, and confidence.
Utilizes the software systems for service plans, client admissions and discharges, scheduling support, client billing and Associate payroll.
Participate in developing, maintaining, and updating written policies and procedures and standards which guide the services provided by the Home Services Associates. Participate in developing, maintaining and implementing the Quality Improvement Programs for Home Services by utilizing input from clients, Care Partners, family and others and evaluating systems and service delivery.
Provide client services as needed to both residents and wait list members as deemed necessary.
Maintain contact with assigned members and/or their families based on the applicable member contact schedule and follow-up as appropriate including advocacy and health education for members.
Using current clinical screening tools and assessments (including member self-health assessments), the Care Coordinator will determine members' appropriateness or eligibility for: a) preventive care initiatives; b) community resources; c) other insurances such as Medicare and long-term care insurance, if applicable
Encourage members in accessing resources to help achieve their health and wellness goals. Resources may include the Connection, online resources or specially developed materials from various community health and wellness resources.
Requirements
Degree in allied health or nursing
Must have a minimum of 1 year experience in providing community-based services
Must have a minimum of 2 years experience managing or supervising care
Demonstrates the ability to problem solve effectively
Able to keep calm in emergency and stressful situations
