Physical Therapist - Park View Post Acute
Position: Full‑time Physical Therapist in Santa Rosa, CA at Flagship Therapy’s Park View Post Acute facility.
Base pay range: $60.00 – $75.00 per hour (higher hourly rate for weekend availability). Flexible schedule with work‑life balance.

Responsibilities
Screen, evaluate, and diagnose patients with physical functioning disorders and/or wound care.
Develop and implement appropriate plans of care following regulatory and clinical practice standards.
Provide rehabilitative, skilled, and medically necessary treatment interventions per regulatory requirements.
Administer various procedures including manual techniques, ambulation, therapeutic exercises, modalities, assistive devices, and wound care.
Consult and counsel patients, families, caregivers, and other service providers regarding physical disorders.
Provide comprehensive discharge summaries per regulatory requirements.
Qualifications
Licensed Physical Therapist.
Open to PTs at all experience levels.
