Job Summary The Clinical Pharmacist’s primary responsibility is to deliver optimal medication management across all dimensions of care. The role ensures safe medication use, meets legal and accreditation requirements, and complies with all applicable policies and standards. Responsibilities include interpreting and evaluating the validity and safety of orders and prescriptions, integrating core clinical pharmacy knowledge with dispensing and order review activities, and performing core clinical activities such as order entry, pharmacokinetics, IV to PO conversion, therapeutic interchange, and other required tasks. Multi‑disciplinary rounds, direct patient care, and educational activities are performed, and the pharmacist serves as a preceptor to interns, technicians, and ASHP‑accredited Residents.

Degrees & Qualifications

Bachelor’s Degree in Pharmacy or Pharm. D. from an accredited school of Pharmacy

Two (2) years of progressive relevant experience in an OP/Retail (for ACS positions) or inpatient hospital setting (for IP positions). PGY1 ASHP‑accredited residency is highly preferred.

License & Certifications

Texas Pharmacist License (or eligibility to obtain one); must be licensed within 90 days of employment.

Texas Pharmacist Preceptor Certification within 180 days of employment.

ACPE IV Sterile Certification.

American Heart Association Basic Life Support (BLS) certification within 90 days of employment.

Advanced Cardiac Life Support (ACLS) with an AHA‑accredited course within 6 months of employment for inpatient positions.

Board of Pharmacy Specialties Certification is preferred and must be maintained; lapsed certs require renewal within six months.

Communication Skills & Attributes

Writing and reporting proficiency in MS Word and PC applications.

Analytical, design, mathematics, medical terms, research, and statistical knowledge.

Ability to work weekends, holidays, flexible shifts, overtime, travel, and on‑call as needed.
