Responsibilities

Provide education related to diabetes and the disease process, medication, diet and nutritional monitoring, foot care, prevention and care of complications, and blood glucose monitoring.

Contribute to the development and implementation of educational programs for patients, families, and staff.

Advise patients on long‑term complications.

Assist patients in monitoring blood glucose for the purpose of medication control and adjustment of insulin and non‑insulin‑dependent diabetics.

Assess, monitor, implement, and evaluate treatment plans for patients at various stages of the disease with a focus on prevention of complications and management of stable diabetes.

Provide ongoing assessment and recommendations to the treatment team and patient/family/support systems for adjustments in the plan of care.

Document assessments, interventions, and plans including recommendations and goals, progress notes, and discharge summaries.

Collaborate with individual patients and groups on setting goals and objectives within the diabetes care plan and use motivational interviewing techniques to promote glycemic control and overall health.

Lead group educational sessions.

Contribute to the development of curriculum and content for clinical staff related to diabetes.

Facilitate training.

Actively participate in educational and sharing opportunities outside the organization to develop expertise.

Propose changes to curriculum that would improve care delivered to our patients.

Provide clinical resource and consultation to staff and clinicians.

Support the organization's commitment to patient and family‑centered care.

Develop and maintain respectful, cooperative working relationships to contribute to delivery of optimal care to our patients and families.

Document plan of care to address the patient's medical condition.

Maintain continuity of patient care assignment.

Complete all Patient Health Information (PHI) forms.

Maintain patient medical records.

Ability to work in a collaborative practice model and willingness to assist with patient care when required.

Capability to adapt quickly to a dynamic work environment.

Strong health assessment/clinical skills.

High degree of accuracy and attention to detail.

Participate in performance improvement and continue quality improvement activities.

May assist and/or perform chaperone duties for patient medical procedures as needed.

Qualifications Education Bachelor’s in Nursing preferred.

RN Licensure required.

Certified Diabetes Care and Education Specialist (CDCES) or eligible to sit for examination within 12 months.

Experience At least 2 years of experience in a healthcare setting, preferably in diabetes prevention or health education program.

Special Skills Current knowledge in diabetes including anatomy and pathophysiology of diabetes and associated complications. Ability to design, implement, and evaluate health program policies and procedures. Excellent leadership, organizational, communication, and problem‑solving skills.

Accountability Will be accountable for following policies and procedures and maintaining relevant knowledge, skills, and leadership competence through continuing education. Responsible for engaging patients and families and providing education on diabetes.

Complexity Requires considerable autonomy in leading projects. Must be able to analyze and plan a diabetes program to improve care, understand patient and family motivation, and engage in a non‑judgmental manner with patients and families.

Licensure/Certification RN Nursing License.

Position Overview Provides state‑of‑the‑art diabetes education in the inpatient setting for persons with diabetes and their families/care partners to optimize diabetes management. Participates in the development of educational materials focusing on disease process, medication management, diet, prevention, and care complications, and blood glucose monitoring. Acts as a health consultant by providing information on the management of diabetes for all patients of ages from infancy to geriatrics. Collaborates with treating physicians in patient care.
