Job Summary The Human Development Institute (HDI) at the University of Kentucky is Kentucky’s Center on Disability. HDI is seeking to fill a Nurse Coordinator. This position is through a contract and is within DBHDID/1915iRISE initiative.

This Nurse Coordinator position will work as a Nurse Coordinator/UM-LOC (Utilization Management/Level of Care) and the responsibilities include, but are not limited to, the following:

Responsibilities

Review and follow up of level of care assessments and person‑centered service plans.

Provide technical assistance to providers as necessary and appropriate.

This position will work remotely and is open to applicants in all areas of Kentucky. Regular work‑related travel throughout the state of Kentucky is a requirement of this position.
