Nurse Coordinator . This position is through a contract and is within the DBHDID/1915iRISE initiative. The Nurse Coordinator will work as a
Nurse Coordinator/UM-LOC (Utilization Management/Level of Care)
and will work remotely, with regular work‑related travel throughout the state of Kentucky. Responsibilities
Review and follow up of level of care assessments and person-centered service plans. Provide technical assistance to providers as necessary and appropriate. Qualifications
Education: Advisable Degree in Nursing (ADN) or higher. License/Registration/Certification: Registered Nurse (RN). Experience: Minimum 8 years related experience. Physical Requirements: Able to accommodate essential functions in a variety of ways. Shift: Monday‑Friday, 8:00 am‑4:30 pm. Travel: Regular work‑related travel throughout Kentucky required. Remote work: Position is remote, open to all areas of Kentucky. Supervisory responsibilities: No. Contact & Application Deadline
