Chronic Disease Prevention Specialist II – Bilingual (English/Spanish)
Salary: $6,252 – $8,540 per month

Position Summary The Public Health Bureau is seeking a dedicated and energetic full‑time Chronic Disease Prevention Specialist II for the Postpone Program. Postpone is a comprehensive youth health program. The CDPS II will support the delivery of group‑level interventions at youth‑serving organizations such as schools and juvenile detention centers, focusing on sexual health education, teen pregnancy prevention, healthy relationships, substance use prevention and overall youth wellbeing. Using evidence‑based program models, the CDPS II will help educate youth on abstinence and contraception, support healthy decision‑making, and promote positive relationship and lifestyle choices. The program’s goal is to reduce adolescent birth rates, substance use, and sexually transmitted infections while enhancing overall wellbeing. The CDPS II will be responsible for activities such as conducting outreach to students, parents, and community partners; delivering curriculum‑based lessons to youth; supporting social media education efforts; providing referrals and resources to youth as needed; assisting with program planning, evaluation, and reporting; and maintaining accurate records and documentation.
Responsibilities
Instruct clients on preventative measures and proper health care procedures.
Develop training aids and provide educational/training sessions to individuals and groups regarding the availability and specific services provided by health care programs.
Review publications for educational suitability, maintain and distribute health education material, and prepare visual aids and display materials.
Assist in publicizing health care programs through media events.
Maintain epidemiological records of clients, contacts, and suspects.
Provide input to management during the planning, development, and implementation stages of chronic disease prevention and health promotion programs, and provide administrative assistance during the administration and evaluation stages.
Assist in local and state health care studies and research projects.
Conduct outreach to students, parents, and community partners; deliver curriculum‑based lessons to youth; support social media education efforts; provide referrals and resources to youth as needed; assist with program planning, evaluation, and reporting; and maintain accurate records.
Qualifications
Principles and techniques used in health care and health promotion programs.
Principles and techniques used in developing training materials.
Knowledge of group dynamics.
Ability to establish and maintain effective work relationships with diverse socioeconomic and cultural groups.
Ability to work under limited supervision in field settings.
Clear oral and written communication skills; ability to write concise reports; effective speaking before groups.
Interview skills, obtaining comprehensive and accurate information in office and field settings.
Education and Experience Any combination of training, education, and/or experience that provides the knowledge, skills, and abilities required should qualify. Examples include:
Completion of all coursework for a four‑year degree in Public or Business Administration, Social Work, Health Education, Behavioral Science, or a closely related field.
Two years of training, education, and experience substantially demonstrating a working knowledge of chronic disease prevention or health promotion programs.
Conditions of Employment
Successful completion of a background check, including fingerprinting.
Possession and maintenance of a valid California Class C driver’s license, or approved alternate transportation.
Availability to work a flexible schedule, including evenings, weekends, holidays, and during disasters or emergencies.
Proficiency in speaking, reading, and writing in English and Spanish.
