Travel MDS Coordinator (RN)
State of Wyoming
Evergreen Healthcare Group is hiring a Travel MDS Coordinator to support skilled nursing facilities across the state of Wyoming.
What You’ll Do:
Completes the MDS, CAAs, and Care Plan per the most current edition of the MDS 3.0 RAI Manual, using facility software (e.g., PCC).
Coordinates the collection of data for each patient, including documentation review, observation, and interview of patient and staff, to assure good standard of practice.
Coordinates and provides an interdisciplinary schedule for MDS assessments and care plans as required by OBRA and PPS.
Reviews provider documentation to complete accurate ICD‑10 coding and ranking as part of the RAI process.
Uses knowledge of Significant Change criteria, Quality Measures, PDPM, and Case Mix to assist the IDT in decision‑making for additional MDS assessments.
Provides education to the IDT on MDS coding and completion, and communicates the need for additional education with DNS and regional teams.
Works in various centers and states (where licensed) to support teams during MDS vacancy, vacation/time off, and other periods of increased needs.
Requirements:
Current RN license in the state of Wyoming (active, unencumbered).
3+ years MDS 3.0 experience.
3+ years PDPM experience.
Proficient in EMR systems (PointClickCare preferred).
