About the job The Transportation Scheduler Dispatcher, under the supervision of the Transportation Manager, is responsible for the timely scheduling of transportation for Senior Community Care (SCC) participants to and from the Day Center and/or appointments. The Scheduler also schedules outside medical appointments for the SCC participants and notifies participants, their families, and home care providers of appointments. The Scheduler communicates any safety or scheduling issues to the Transportation Manager.
Required Qualifications
High school diploma or equivalent required.
Minimum one year of experience working with the frail elderly.
Must pass a background check and drug screen.
Valid Driver’s License in state of hire.
Must have medical clearance for communicable diseases and up-to-date immunizations before having direct participant contact.
Skilled in establishing and maintaining effective working relationships with participants, caregivers, drivers, and co-workers.
Essentials
Familiar with PACE Care Scheduling Program to gather information needed for timely and efficient transportation and home care schedules.
Considers the needs of the participant and requirements of the PACE Center, and communicates changes to the Transportation Manager.
Develops schedules to limit employee overtime hours as much as possible.
Schedules Transportation for participants to and from the Day Center, medical/specialist appointments, activity outings, and last‑minute emergencies as requested by the Interdisciplinary Team via the plan of care.
Assists in scheduling vehicle maintenance and equipment to operate within manufacturer recommendations.
Functions as a member of the Interdisciplinary Team when appropriate.
Communicates with Transportation Manager daily to stay updated on participant conditions, plan of care changes, or Interdisciplinary Team requests.
