Social Worker - Geriatrics and Extended Care U.S. Department of Veterans Affairs

Summary The Social Worker is assigned to a Community Living Center (CLC) at the Lake City VA Medical Center within the North Florida/South Georgia Veterans Health System (NF/SGVHS) and is aligned under Social Work Service. The position can also float and cover other areas, including medical surgical units, PACT, and Emergency Department.

Responsibilities and Duties

Complete assessments, facilitate Goals of Care conversations, enable the completion of advance directives, discharge planning, and provide crisis intervention services.

Provide ongoing behavioral health interventions to Veterans and their families at the Lake City VA Community Living Center neighborhoods.

Offer short‑term solution‑focused or problem‑solving focused evidence‑based interventions, support groups and counseling/psychotherapy.

Collaborate with interdisciplinary treatment teams and Veterans or their caregivers.

Conduct psychosocial assessments, formulate and implement treatment plans, evaluate outcomes, and coordinate all phases of continuing care.

Act as an advocate with VA and community service providers/agencies; maintain relationships with community agencies and networks.

Assist with crisis assessment, develop, and execute crisis management plans, and negotiate crisis situations using supportive‑problem‑solving or crisis intervention techniques.

Collaborate in performance‑improvement processes and comply with performance measures required by the VA.

Float to other areas such as medical surgical unit, PACT, or Emergency Department as needed.

Qualifications

Citizenship

– United States citizenship; non‑citizens only if no qualified citizens are available.

Proficient in written and spoken English.

Master's degree in Social Work from an accredited school (CSWE). Doctoral degree is not a substitute.

Licensed or certified to practice at the advanced practice level; includes advanced generalist or clinical examination. Grandfathered licenses allowed except for California.

Experience: Minimum of one year of post‑MSW experience equivalent to GS‑9 in healthcare or related settings.

Knowledge, Skills, and Abilities (KSAs):

Use a Veteran‑centric approach when providing interventions and counseling.

Assess psychosocial functioning and needs, formulate and implement treatment plans.

Implement treatment modalities with individuals, families, and groups.

Establish effective working relationships with clients, staff, and community agencies.

Fundamental knowledge of medical and mental health diagnoses, disabilities, treatments, medications, and terminology.

Preferred: Basic Life Support (BLS), Advanced Cardiac Life Support (ACLS), or Advanced Trauma Life Support (ATLS) certification; at least two years in a specialized social work practice.

Physical Requirements: Able to perform essential functions with or without reasonable accommodation.

Pay Range Base pay: $61,111.00 / yr – $96,116.00 / yr (GS‑9 to GS‑11). Pay varies based on skills and experience.

Benefits and Work Schedule • Monday – Friday, 8:00 am – 4:30 pm
