The Social Worker - PACT/Specialty is assigned to the Jacksonville University Blvd Clinic within the North Florida/South Georgia Veterans Health System (NF/SGVHS). NF/SGVHS is a highly complex level 1a facility. NF/SGVHS consists of 2 medical centers, 3 large outpatient clinics, and 9 community-based outpatient clinics across 50 counties, of which 19 are in Southern Georgia. NF/SGVHS serves approximately 135,000 unique Veterans each year.
Duties The North Florida/ South Georgia VA Healthcare System Social work Service is seeking an experienced and dynamic Social Worker- Patient Aligned Care Team (PACT)/Specialty at the Jacksonville Clinic. Ideal applicants are those who want to contribute to the achievement of quality care standards, as well as the growth and development of a VA academic hospital program. You must be an excellent communicator and team player who is adaptable to the needs of a growing medical center. The Social Worker - Patient Aligned Care Team (PACT) performs comprehensive psychosocial assessments, discharge planning, counseling with individuals/families/groups, provides education to Veterans/families, coordinates and arranges community based services, facilitates VA non-institutional care services, participates in multidisciplinary teams, provides advocacy for Veterans, provides consultation and education to staff regarding psychosocial aspects, and represents the mission of the VA in all interactions.
