Nurse Transplant Coordinator/UKHC Requisition Number: RE52672 Working Title: Liver Transplant Nurse Coordinator Department Name: H3420:Transplant Work Location: Lexington, KY Grade Level: 11 Salary Range: $29.12-45.70/hour Type of Position: Staff Position Time Status: Full-Time Required Education: ADN Required Related Experience: 3 years Required License/Registration/Certification: Registered Nurse (RN)
Physical Requirements: Ability to lift, carry, push and pull 25-100 pounds daily; regularly sit at a computer workstation for extended periods with repetitive motions such as typing; stand, walk and sit for long periods. Exposure to communicable diseases, injury from patients or sharp instruments may occur. Shift: Primarily day shift, Monday through Friday, with on‑call shift rotations as needed.
Job Summary: The Transplant Nurse Coordinator coordinates new patient referrals, evaluation, listing and transplant care management for both single‑organ and multi‑organ patients. The Coordinator collaborates with multidisciplinary team members to provide individualized patient care and participates in quality improvement efforts aligned with program, department, and hospital strategic plans. They serve as the liaison and initial professional point of contact for providers and patients.
Skills / Knowledge / Abilities: Excellent nursing care skills; self‑starter mentality; organizational skills; comprehensive knowledge of related disciplines; ability to prioritize; basic to proficient computer/technical skills, including text processing, spreadsheet manipulation, database processes, and Outlook proficiency.
Preferred Education/Experience: BSN, transplant experience, and Certified Clinical Transplant Coordinator (CCTC) certification preferred.
