MGR - ASSESSMENT/REFERRAL (BH)

Responsibilities We have an immediate opening for an Assessment and Referral Manager for our intake department in the Jacksonville Market. This position provides managerial oversight of the timely processing of inquiry and referral calls for two(2) hospitals, River Point and Wekiva Springs. Ensures Department is effectively communicating with referral sources, physicians, unit staff and hospital personnel involved with each transfer request. Monitors the entry of call data into the facility’s database and manages all e‑fax application(s) to process incoming referrals and transfer requests in a timely manner. Ensures Department effectively communicates with unit staff, physicians, and Admissions Triage Staff. Assists with development of and ensures compliance with hospital’s policies and procedures regarding referral management. Maintains Department schedule and leads the hiring, interviewing, and training of new Staff. Assists Admissions Director with the analysis of referral and key indicator data.

This position requires a Master’s Degree from an accredited college or university in social work, psychology, mental health, nursing or a related field. For RN’s, licensure as a Registered Nurse in the state of Florida and 3‑5 years of experience preferred in psychiatric health care. Minimum of 1‑3 years of experience managing a Clinical team.
