Emergency Department Social Worker
This position is responsible for serving as a member of an interdisciplinary team providing in-patient and out-patient social work services to patients and their families who present with losses related to illness, injury, or hospitalization. Interventions include biopsychosocial assessments, group work and care conference facilitation, crisis/brief counseling, and discharge planning with community resource coordination.
Responsibilities
Provide biopsychosocial assessments of patient and family situations.
Document assessments and interventions in the patient’s medical record.
Develop written care plans and evaluate outcomes for home health and hospice patients.
Assist the health care team in understanding cultural and socio-economic factors.
Perform supportive counseling for patients and families dealing with disability, illness, loss, and bereavement.
Assist patients/families in utilizing relevant community resources.
Build customer relations with patients, colleagues, and community referral sources.
Ensure personal and professional development to maintain current knowledge.
Manage and perform assigned duties efficiently and professionally.
Apply clinical social work interventions to in-patient and out-patient areas as required.
Perform other duties as assigned.
Minimum Education / Work Experience
Bachelor’s degree in social work.
Knowledge / Skills / Abilities
Clinical, organizational, assessment, and communication skills.
Ability to meet background check requirements.
Effective written and verbal communication in English.
Ability to provide patient care appropriate to the age and needs.
Ability to work independently and provide effective individual, departmental, and organizational communication.
License / Certifications
Licensed by the Minnesota Board of Social Work as an LSW, LGSW, LISW, or LICSW.
Preferred Qualifications
Master’s degree in social work.
Practical experience in a health care setting.
