Job Summary The candidate will perform duties across multiple counties in Georgia. The position is field‑based, supervised by the Regional Field Office.
Responsibilities include:
Assisting individuals prioritized for NOW or COMP waivers with waiver service entry, completing required documentation, and maintaining knowledge of local resources for support‑needed individuals.
Communicating with individuals and families to gather required information and maintain awareness of changing circumstances, level of need, and specific support needs.
Participating in regional planning list meetings to support team members and improve knowledge.
Assisting families, including those moving into waiver services, with the Medicaid application process as necessary.
Helping individuals with IDD choose service providers by providing information, facilitating meetings, or scheduling provider fairs, and developing a network of community contacts to educate families and make referrals for non‑waiver supports.
Maintaining close contact with planning list individuals who have significant health needs and assisting families in accessing all support available through the Medicaid state plan.
Complying with HIPAA and DBHDD policies regarding confidential health information.
Reporting concerns of abuse, neglect, exploitation, or immediate risk per policy.
Assisting with State Service Coordination tasks as needed.
Preferred Qualifications - External
Experience or knowledge of individuals with intellectual/developmental disabilities, Medicaid waivers, and DBHDD I/DD services; ability to work independently without daily supervisor contact.
Spanish fluency.
Employment Requirements Licensure & Evaluations:
The following may be required for all employees, including those not involved with direct care: Drug Screening, TB Evaluation, Annual Influenza vaccination (exemptions may apply for medical or religious reasons).
Required Qualifications
Bachelor’s degree in a related field and two (2) years of job‑related experience; or two (2) years of experience at the lower level Social Services Program Coordinator 1 (SSP030) or equivalent position.
Eligibility for licensure where required.
Background check as required by state law.
