Under the direction of the Supervisor or Manager, the Care Integration Coordinator supports the Care Management team by coordinating care for members/patients throughout the healthcare continuum. This includes performing clerical tasks related to team functions and activities identified during initial and ongoing assessment and management.
Responsibilities include:
Support Care Management team in coordinating member/patient care across the continuum.
Perform clerical tasks related to team functions and activities identified during assessment and management.
Required Qualifications:
1 year of healthcare experience.
Preferred Qualifications:
Associate’s Degree in a related field.
Medical Assistant certification (upon hire).
Managed care or case management experience.
Experience with Mcg Care Guidelines, Touchwork, IDX, Meditech.
Experience with Allscripts, Touchwork, Enterprise, or IDX.
