Longevity Health is a clinical services company that optimizes the quality of life for people in long-term care by focusing on individualized health needs. Founded in 2018, we provide a national Institutional Special Needs Plan (ISNP) and operate in 13 markets with a focus on value-based partnerships with nursing homes and other long‑term care facilities.

About the Role The Member Advocate is a key member of the LHP team, responsible for building strong relationships with skilled nursing facility (SNF) staff and ensuring the well‑being of our members. This role combines member advocacy with the coordination of our music‑based benefit, designed to enhance the lives of residents through meaningful engagement.

Key Responsibilities

Facilitate interactive, social music groups that include music, movement, and trivia to engage residents’ minds and bodies (no music experience required).

Meet members one‑on‑one to develop relationships, assist with ordering health‑plan supplies, and maintain conversation.

Develop and maintain relationships with care‑facility activities and clinical staff.

Travel to assigned facilities on a scheduled basis (mileage reimbursement provided).

Work from home as needed and maintain a flexible schedule.

Qualifications

High School Diploma or GED; Associate’s or Bachelor’s degree preferred.

Positive, upbeat personality with excellent communication skills.

Desire to help support a vulnerable population.

Ability to facilitate group music sessions using provided materials.

Willingness to travel between assigned facilities within the territory.

Valid auto insurance coverage as required by state laws; new employees must provide proof prior to hire and ongoing proof.
