Job Summary As a Facility Investigator II, you will perform moderately complex investigative work and administrative tasks at the State Supported Living Center (SSLC). Responsibilities include field investigations of Abuse, Neglect, and Exploitation (ANE) and injuries involving residents; conducting comprehensive preliminary investigations; following up on external cases such as Health and Human Services Commission Provider Investigations (HHSCPI) and the Office of Inspector General (OIG); ensuring compliance with ICF and Settlement Agreement requirements; verifying evidence; summarizing findings; preparing reports; presenting testimony at administrative hearings or court proceedings; serving on the Incident Management Team; conducting Root Cause Analysis meetings; and providing technical assistance as needed.

Essential Job Functions (EJFs) Attending work on a regular basis and maintaining a specific shift or rotating schedule as required. Conducting investigations of reportable incidents and serious injury; collecting and reviewing evidence; interviewing witnesses; summarizing investigative findings; preparing reports for supervisors; presenting cases at hearings; attending daily meetings related to ANE and unusual incidents; serving on incident and root cause analysis teams; collaborating with external agencies including HHSC, law enforcement, and OIG; completing data entry for unusual incidents; maintaining incident data for trend analysis; facilitating audits and inspections; assisting with administrative duties such as ordering supplies and scheduling interviews; and performing disaster response or continuity of operations as assigned.

Knowledge, Skills and Abilities (KSAs) • Knowledge of state and federal laws, ICF‑IID regulations, and protective services procedures.

• Understanding of laws and policies related to protection from harm and residential rights.

• Familiarity with investigative principles, techniques, and procedures.

• Proficiency with computer applications (Word, Excel, Outlook).

• Interviewing and negotiation skills.

• Record searching capabilities.

• Ability to formulate comprehensive investigative reports.

Initial Screening Criteria • Graduation from an accredited four‑year college or university with major coursework in criminal justice or a related field (preferred but not required).

• Graduation from an accredited high school or GED.

• Experience in investigative or incident management work.

Additional Information Applicants must pass a fingerprint criminal background check, pre‑employment drug screen, and registry checks including CANRS, Nurse Aide, Medication Aide, and employee misconduct and HHS List of Excluded Individuals/Entities (LEIE). Males aged 18–25 must register with the Selective Service. All employees are subject to random drug testing. Flexibility in work hours may be required, including overtime, extended hours, weekends, and holidays.
