Base pay range $57,764.00/yr - $72,630.00/yr
Overview Habilitation services play a crucial role in assisting individuals in acquiring, retaining, and improving self-help, socialization, and adaptive skills. The Habilitation Specialist 1/Process Owner will support the Quality Management Department by ensuring that documentation for Home and Community-Based Services (HCBS) meets the required standards set forth by Medicaid and agency policies. The role includes monitoring compliance, providing training, and fostering collaboration with various stakeholders to ensure quality service delivery.
Key Responsibilities
Documentation Review – Review and monitor HCBS documentation to ensure it aligns with Medicaid and agency requirements, focusing on accuracy, completeness, and compliance.
Training Provision – Develop and deliver training sessions for Habilitation Specialists on Quality-of-Life Plans, Staff Action Plans, Safeguard Summary Plans, Monthly Summary Notes, and other relevant documentation.
Billing Discrepancy Identification – Conduct regular reviews of the Electronic Health Record system to identify and resolve billing discrepancies.
Observation of Documentation – Conduct observations to verify the implementation of person-centered goals in accordance with the Life Plan and Staff Action Plan.
Relationship Building – Establish and maintain effective working relationships with Habilitation Specialists, Care Managers, Care Coordination Organizations, Consumer Advisory Board (CAB) advocates, and the Medicaid Compliance Liaison.
Tracking System Creation – Develop a tickler system for tracking the distribution of Life Plans and Staff Action Plans, ensuring concerns noted during Life Plan meetings are reconciled and followed up on as necessary.
Minimum Qualifications
Bachelor’s or higher-level degree in a human services field* (list of included disciplines).
One year of permanent competitive or 55-b/c status in the posted title and be eligible for lateral transfer; OR be eligible to transfer via Section 70.1, 70.4, or 52.6 of Civil Service Law.
Competitive: Candidates must be reachable for appointment on the current Civil Service Eligible List for this title.
Additional Comments
Title is part of NY HELPS program; may be filled via a non-competitive appointment.
At a future date, employees hired under NY HELPS expected to have their permanent non-competitive employment status converted to permanent competitive status.
