MDS Coordinator / RN – Skilled Nursing Compliance & Care Planning Join The Grand Healthcare System as an MDS Coordinator / RN in Altamont, NY, overseeing resident assessments, care planning, and regulatory compliance.

Base Pay Range $90,000 – $110,000 per year.

Location & Schedule

Altamont, NY (25 minutes from Albany)

Full‑time, Monday–Friday 8:00 am–4:00 pm (flexible)

Position Summary The MDS Coordinator is responsible for completing accurate, timely resident assessments and implementing care plans in compliance with Medicare and Medicaid standards. Collaborate with interdisciplinary teams to ensure quality outcomes, support staff, and communicate effectively with families.

Primary Responsibilities

Complete timely and accurate assessments of resident care using MDS protocols.

Assist in identifying resident needs and communicate care expectations to families.

Support development and implementation of interdisciplinary care plans.

Monitor care plan effectiveness and compliance with regulatory standards.

Collaborate with social services, dietitians, rehab specialists, and medical staff.

Maintain documentation to ensure accurate reimbursement and care outcomes.

Qualifications

Current NYS RN license.

Prior experience as an MDS Coordinator in long‑term care.

Strong understanding of Case Mix Index (CMI) and ability to maximize CMI.

Knowledge of Medicare and Medicaid regulations.

Excellent organizational, analytical, and communication skills.
