Position Purpose The Intake Coordinator is responsible for overall admissions to the Crisis Stabilization Unit (CSU). The role implements efficient admission procedures, evaluates medical necessity, documents intake and treatment plans, and coordinates care through the full admission‑to‑discharge continuum. The Coordinator works on site 4 days a week (10‑hour shifts) plus one weekend shift; 5‑to‑8‑hour shifts are also an option.
Essential Duties & Responsibilities
Complete required intakes for all youth admitted to the CSU within 24 hrs, ensuring compliance with CDHS and BHA rules.
Develop initial treatment and safety plans with the client.
Outreach to parent/guardian to explain program expectations and obtain treatment goals.
Serve as liaison among stakeholders to facilitate professional communication and problem‑solving.
Accurately enter client information into the electronic health record system.
Assemble client charts and distribute intake data to the CSU team.
Identify risk factors (e.g., suicidal, homicidal, grave disability); complete involuntary holds and assist with commitment protocols.
Conduct behavioral and diagnostic assessments (PHQ-9, GAD-7, child trauma screener) and manage engagement strategies and case management.
Coordinate care with internal and external providers to achieve agreed outcomes.
Obtain collateral information from family and other providers as needed.
Adhere to mandated reporting requirements.
Participate in interdisciplinary team meetings and daily rounds for treatment and discharge planning.
Assist with discharge planning and completion of discharge summaries and related tasks.
Maintain professional ethics and comply with all Jefferson Center policies.
Attend and complete all required training, including CPI, CPR/First Aid, and trauma‑informed care.
Additional Duties (Performance Measures, Professional Growth, Relationships)
Attend mandatory in‑services and comply with individualized training plans.
Prepare for and actively participate in supervision, reporting high‑risk cases, and problem‑solving.
Maintain compliance with regulatory and documentation standards.
Submit Progress‑to‑Date forms (≥90 %) before shift end.
Complete paperwork (≥95 %) by due date.
Demonstrate adaptability, courtesy, flexibility, and cooperation.
Build therapeutic alliances with diverse clients and uphold cultural competence.
Participate in staff development to enhance professional growth.
Ensure appropriate healthcare releases for physical needs and coordinate referrals.
Conduct assessments within scope of expertise and apply clinical interventions accordingly.
Apply trauma‑informed care principles to reduce re‑traumatization.
Promote a supportive, collaborative team environment.
Maintain health standards; the role may involve lifting up to 25 lbs and assisting in holding individuals up to 60 seconds in rare situations.
Adapt to staffing needs by deploying to other units or teams when required.
Required Education, Knowledge, Skills & Experience
Master’s Degree from an accredited institution plus two years of relevant experience or training.
Bilingual (English/Spanish) preferred.
CPR/First Aid Certification preferred or to be obtained within two months of hire.
Minimum age 21; 25 years or older required for vehicle insurance coverage and acceptable Motor Vehicle Record.
Salary Range $57,200 – $61,360 per year ($27.50 – $29.50 per hour). 1.0 FTE (40 hrs/week).
Less than 40 hrs/week will be prorated.
Bonuses $1,000 at 1 month anniversary, $1,000 at 6 month anniversary, $500 at 1 year anniversary (Total: $2,500).
Minimum 20 hrs/week required for eligibility; not earned until continuous employment reaches the anniversary date.
