Qualifications

Must be a QMHP in accordance with Medicaid Provider Manual Guidelines.

Possess specialized training (including fieldwork and/or internships associated with the academic curriculum where the student works directly with persons receiving mental health services as part of that experience) OR one year of experience in treating or working with a person who has mental illness; AND

Be a human services professional with at least a bachelor's degree in a human services field

Duties and Responsibilities

Demonstrate the ability to engage individuals in a welcoming, hopeful, empathic manner regardless of disability or phase of recovery.

Screen for co-occurring disorders and recognize diagnostic criteria used to identify substance abuse or dependency.

Assess and evaluate the needs of individuals and continue caseload contact to develop goals. Demonstrate the ability to identify stage of change and use interventions consistent with stage of treatment. Assist clients in developing strength-based, stage‑wise treatment matched person‑centered plans that are designed to address identified needs. Complete referral forms, clinical and legal documentation necessary to obtain community-based services for clients as well as collaboration and communication with other SA/MH community partners.

Advocate and provide options for supports and services as needed and seek out and develop community resources to meet the needs of assigned individuals for activities and support services.
