Case Management Coordinator – Specialized Residential Services

Qualifications
Must be a QMHP in accordance with Medicaid Provider Manual Guidelines
Possess specialized training (including fieldwork and/or internships) OR one year of experience treating or working with individuals with mental illness; AND
Hold at least a bachelor's degree in a human services field
Duties and Responsibilities
Assess and evaluate the needs of individuals, develop goals, and assist with program plans to meet identified needs. Collaborate with Residential Providers and educate provider staff on Treatment Plans. Complete referral forms, clinical, and legal documentation to secure community‑based services. Advocate for new services and seek community resources to support individuals.
Provide case management services for individuals, families, and/or guardians for support, rehabilitation, and crisis intervention. Monitor and evaluate placements in group homes, in‑home supports, schools, or programs to ensure consistent treatment and progress. Assist in commitment procedures when necessary, as well as voluntary admissions.
Ensure discharge planning activities are performed within the 72‑hour requirement for individuals released from a hospital.
Maintain paperwork in accordance with Medicaid, Department of Community Health guidelines, and Easterseals MORC policies and procedures.
