Advanced Practice Practitioner – Health at Home Hospice Team
Full‑time: 40 hours per week
Shift: Day – 8:00 AM to 6:30 PM, 4 days per week (no weekend, holiday, or on‑call except occasional backup of medical director)
Location: Fairhaven, New Bedford, MA
Responsibilities
Under direction of the Director of Supportive Care Services or designee, work in collaboration with a physician and other palliative care team members, in an active practice to deliver hospice care and related health care services to patients and families
Perform a wide variety of duties including, but not limited to: bedside patient/family evaluation; assisting with diagnosis, treatment and management of acute and chronic health conditions; educating, counseling patients, families and/or caregivers as to preventative care, medical problems, etc; and serving as a nursing resource for education and consultation to members of the interdisciplinary team
Qualifications
Requires a Master of Science Degree in Nursing
Certification from a national accrediting body to practice as a Nurse Practitioner is required
Current RN license in the Commonwealth of Massachusetts and Rhode Island is required
Minimum of 5 years nursing experience with clinical nursing care in oncology, hospice, geriatrics, and/or intensive care preferred
A minimum of one‑year Nurse Practitioner experience
Certification in Hospice and Palliative Care required
Must possess current CPR certification
Pay Range USD $62.50 – $71.00 per hour
Rotation Schedule Requirement Occasional on‑call to back up medical director – no scheduled on‑call responsibility ongoing
