Job Summary
Registered Nurse RN Home Health (PRN) – Empath Home Health – North Hillsborough County (Lutz/University area). This PRN Home Health RN will provide skilled nursing care for patients in their homes throughout north Hillsborough County, including Lutz, New Tampa, Tampa Palms, Temple Terrace, University, Thonotosassa, and surrounding areas. In this role, you’ll deliver individualized care, build trusted relationships, and help patients manage their health with dignity.
Location: Home health visits in north Hillsborough County. Schedule: PRN role offering 10+ visits per week with required weekend/holiday on‑call rotations; potential for full‑time in the future.

What You’ll Do
Perform comprehensive patient assessments, including OASIS assessments at required intervals
Develop, initiate, and revise individualized plans of care as patient needs evolve
Deliver skilled nursing services and treatments as outlined in the plan of care
Counsel and educate patients and families on disease processes, self‑care techniques, and prevention strategies
Collaborate with the interdisciplinary team to ensure seamless, holistic care

What You’ll Need
Graduate of an approved nursing program with an active Florida RN license
Preferred BSN or MSN degree
Minimum of 1 year of direct patient care experience
Home health RN experience with OASIS documentation strongly preferred
Preferred experience using Homecare Homebase (HCHB) EMR
Strong assessment, critical thinking, and patient education skills
Willingness to participate in weekend and holiday on‑call rotations
