Responsibilities
Conduct thorough patient assessments to identify needs and create individualized treatment plans.
Implement therapeutic exercises, manual therapy techniques, and other interventions to address patient goals.
Educate patients and their families on exercises and activities to improve mobility and function.
Monitor patient progress and adjust treatment plans as necessary.
Collaborate with other healthcare professionals to ensure comprehensive and coordinated care.
Maintain accurate and timely documentation of patient care and progress.
Qualifications
Current state licensure as a Physical Therapist.
Proven experience in a home health setting is preferred but not required.
Strong interpersonal and communication skills.
Ability to work independently and manage time effectively.
Valid driver’s license and reliable transportation.
