STGi is currently seeking a Nurse Practitioner to provide services at our Community Based Outpatient Clinic in Kingston, New York.

Job Summary The Nurse Practitioner is an Advanced Practice Registered Nurse (APRN) who delivers a wide range of health services. Responsibilities include urgent and emergent evaluation of episodic and chronic problems, comprehensive primary health care, prescribing medications, ordering and interpreting diagnostic studies, and patient and family education. The role requires collaboration with the Primary Care Provider (PCP) team and active participation in professional activities at local and state levels.

Essential Functions

Provide primary and preventive medical care following CPT codes and within the scope of training.

Deliver care in accordance with the National Nurse Practice Act, Joint Commission standards, VA Medical Center guidelines, and Inspector General directives.

Collaborate closely with the Patient Affiliated Care Team (PACT) and support staff, including daily huddles.

Emphasize wellness, prevention, and early detection.

Supervise support staff in patient care delivery.

Maintain universal precautions and infection control practices.

Ensure smooth patient flow per VA access standards and triage protocol.

Perform CPR and assist during respiratory and cardiac arrest procedures.

Conduct comprehensive health assessments and diagnostic plans.

Develop, prescribe, and adjust treatment plans based on outcomes.

Manage medication prescriptions within state and federal regulations.

Coordinate specialty referrals and follow‑ups.

Operate therapeutic and emergency equipment (e.g., monitors, EKGs, AEDs).

Document all patient encounters in VISTA, maintaining accurate records.

Complete patient charts within 24‑72 hours and place RTC orders promptly.

Assist with scheduling and patient communication regarding care plans and test results.

Provide health education materials to patients and families.

Ensure compliance with all federal, state, and local safety and operational regulations.

Participate in performance improvement and staff development programs.

Maintain patient confidentiality and support privacy rights.

Support day‑to‑day activity management of the CBOC.

Perform other clinical duties within scope limits.

Required Skills

Three years of clinical nursing experience; at least one year as an APRN (three years preferred).

Experience in outpatient Family or Internal Medicine settings preferred.

Current unrestricted DEA license and active scope of practice/collaborative agreement.

Unrestricted New York nursing license.

BLS/CPR certification (AHA or MTN).

Prescriptive authority per state regulations.

Ability to set realistic, measurable education goals for patients.

Proficiency in individual and group counseling focused on disease prevention.

Experience with safe opioid practices for chronic pain.

Competency with Microsoft Office and general computer skills.

Exceptional communication skills and ability to handle challenging situations.

Detail oriented with strong multitasking ability.

Required Education

Master’s degree in nursing from an NLNAC or CCNE accredited program.

Current Nurse Practitioner certification from the ANA.

ANA certification in either Adult Health or Family Practice.

Working Conditions / Physical Requirements

Moderate physical activity, handling weights up to 25 lbs.

Standing and walking for more than four hours per day.

Interior office environment with readiness to respond to medical emergencies.
