Pay Range Base pay: $44,600 - $61,400 per year.
Additional compensation: $28.00 - $40.00 per visit/unit.
Projected annual range: $53,000 - $73,000 per year.
Responsibilities
Provide professional nursing services under the direction of an RN, in compliance with state Nurse Practice Act and CenterWell policies.
Administer skilled nursing interventions for patient illness, rehabilitative needs, and preventative care, using a holistic approach.
Apply clinical standards to facilitate problem resolution and achieve individualized patient goals and outcomes.
Communicate with RN Supervisor regarding plan of treatment changes and accept verbal orders from physicians permitted by state law.
Utilize systematic, outcome-driven approach in implementing the nursing plan of care.
Maintain timely contact with patients, physicians, clinical managers, and care team regarding patient/family needs and status changes; participate in care coordination and discharge planning.
Maintain professional conduct and confidentiality, sharing information only when permitted and in patient best interest.
Ensure availability of supplies, materials, and equipment required for safe and effective plan implementation.
Prepare, submit, and maintain required documentation; record visit/shift notes as services are rendered.
Qualifications
Graduate of an accredited Licensed Practical Nursing Program or accredited School of Vocational Nursing.
Current nursing license in the practicing state.
Valid driver's license, auto insurance, and reliable transportation.
Current CPR certification.
Two years experience as an LPN/LVN in a clinical setting, preferably home health or hospice.
