Job Summary The Licensed Practical Nurse (LPN) provides direct patient care in a long‑term care or nursing home setting under the supervision of a Registered Nurse or physician. This role supports patient safety, comfort, and continuity of care while adhering to clinical standards and organizational policies.
Key Responsibilities
Deliver safe and compassionate nursing care to residents in a LTC/Nursing Home environment
Administer medications and treatments as prescribed and within scope of practice
Monitor and document patient conditions, vital signs, and responses to treatment
Assist with activities of daily living and patient mobility as needed
Collaborate with RNs, physicians, and interdisciplinary care teams
Maintain accurate and timely documentation in patient medical records
Adhere to infection control, safety, and regulatory standards
Communicate effectively with residents, families, and healthcare team members
Perform additional duties as assigned
Required Qualifications
Active Licensed Practical Nurse (LPN) license (state license required)
Minimum of 2 years of recent LPN experience
Prior Nursing Home or Long‑Term Care (LTC) experience required
Current BLS certification
Strong clinical, communication, and documentation skills
Physical Requirements
Ability to stand, walk, lift, and assist patients throughout the shift
Ability to perform nursing duties in a fast‑paced clinical environment
