Responsibilities
Home visitation with infants and toddlers and their families
Conduct developmental screenings and/or eligibility evaluations
Assist families with the implementation of functional, age-appropriate strategies
Develop Individualized Family Service Plans (IFSP) with families
Provide service coordination to families
Assist child with transition to preschool programming
Work in transdisciplinary team of Early Childhood professionals
Position requires home visitation and regional travel throughout Waterbury
Regular and consistent attendance is considered an essential function of this position
Qualifications
CT DPH Licensed/Certified Physical Therapist
Experience with Infants, Toddlers and Families
Service Coordination training (or willingness to obtain)
Excellent organizational skills and computer literacy
Driver’s license and auto insurance required
Position is open until filled / Anticipated start date is ASAP
