Summary:
Coordinates care, communicates with, and supports patients and their families through the rehabilitation continuum of care for the Brain Injury Program Line. Knowledgeable of brain injury resources available during and post rehabilitation. Demonstrates proficiency, autonomy, and accountability in assuring patient case management needs are met from a well-coordinated interdisciplinary perspective. Maintains understanding of performance measurements for program line. Takes leadership in performance improvement and customer service initiatives. Must be able to demonstrate the skills and knowledge necessary to assess and provide intervention appropriate to the age, education level, and capabilities of the person served.
Other information: Bachelors Degree in Medical Social Work or equivalent is required.
This job will be authorized 80.00 hours bi-weekly.
