Summary
The Genesis Patient Navigator will work as part of a specialized team within the Genesis setting of Sun River Health. Services include but are not limited to: assessing patients' needs and barriers to care; coordinating care with interdisciplinary team; and providing education and support. The Patient Navigator utilizes motivational interviewing and a harm reduction approach to provide trauma informed care based supportive interventions. Performs other duties as assigned.

Sun River Health is a Federally Qualified, Non-Profit Health Center serving communities in Suffolk, Rockland, Orange, Duchess, Ulster, Sullivan, Columbia and Westchester County. Sun River Health is seeking a full-time Genesis Patient Navigator to support our Peekskill site.
Essential Duties and Responsibilities
Assist patients with referrals for specialty care and community resources to address social determinants of health.
Possesses and uses sound and ethical judgment, and has the ability to maintain effective relationships with individuals and organizations.
Assist in coordinating patient psycho-education groups.
Facilitates patient appointments within different departments of SRH and outside agencies. Follows SRH procedures for referring patients to different departments of SRH and outside agencies, utilizing SRH preferred provider lists.
Consult with medical staff to link patients to services through the provision of ancillaries such as translation, transportation assistance and application assistance.
Assist patient in overcoming obstacles that would prevent them from receiving needed referrals.
Answer patient questions to clarify provider instructions, procedures and referral needs.
Review open referrals for upcoming medical appointments and provide to clinical team.
Conduct referral follow up and document outcomes in the EMR consistent with program policies and procedures and standards.
Educate patients on the importance of medication and medical adherence, U = U, harem reduction, and safer sex practices.
Participate in trainings to increase knowledge of HIV/AIDS and related topics, inclusive of the completion of Peer Certification Program.
Demonstrates cultural competency in working with diverse populations using an anti-racist framework and equity lens.
Conducts outreach to (re)engage HIV+ patients in care.
Maintains progress notes and case records as required in EMR and relevant grant-funded data programs.
Accompanies patient to appointments and/or lab services as needed.
Participates in case conference and QI activities as well as other agency or community-based meetings or trainings as assigned.
Works with members of the multidisciplinary care team to help improve patient health outcomes.
Participates in quality improvement trainings and activities to improve patient care.
Requirements
High School Diploma/GED Required
2-4 years of related work experience
