The Case Managerprovides case management for clients who may be diagnosed with chemicaldependency or co-occurring chemical dependency and mental health disorders. TheCase Manager also functions as an advocate and support person to individualsassigned to their program and links them to appropriate services and monitorsclient progress.
JOB DUTIES AND RESPONSIBILITIES
Advocates in the client's best interest.
Conduct client intake, screening, assessment,orientation, and related reporting.
Develop and implement case management plans for clientsthat address goals of obtaining stable housing, gainful employment,engagement in behavioral health recovery support, and linkage to ancillaryservices to support those goals.
Develop transition and discharge plans with clientsthat include provisions for behavioral health disorders.
Coach clients on engagement with treatment plans andself-help recovery services, individualized health and wellness goals, andself-responsibility.
Facilitate life skills group with fidelity to evidencebased practices.
Prepare and maintain clinical records in accordancewith policy and procedure.
Interface positively with community partners.
Participate in client case reviews and team meetings.
Make referrals to appropriate resource agencies anddocument outcomes of referrals.
Provide crisis intervention, when needed.
Maintain client rights as defined by DCF.
Maintain the highest standards in ethical conduct andconfidentiality.
Maintain clinical records in accordance with Agencystandards.
REQUIRED KEY PERFORMANCE INDICATORS
Completetargeted case management direct service hours 1352 hours per year.
ConcurrentDocumentation is expectation and all notes completed same day of service.
MeetRecovery goals through change of level of care within set time frame 90% of thetime.
Will have99% accuracy in E.H.R. on enteringinformation.
100% ofevents provided will be on the active treatment plan.
100% ofrequired trainings will be completed within specified time frames.
No show rate below 10%.
REQUIRED KNOWLEDGE, SKILLS, AND ABILITIES:
Bachelor'sdegree with major course work in human services field.
Minimumof 5 years relevant experience working with adults with substance abuse/mentalhealth issues.
Must have a clear understanding of thecharacteristics and problems associated with mental health diagnosis(es) orco-occurring mental health and substance abuse diagnosis.
Knowledge of substance use disorders and theprinciples of recovery.
Knowledge of Trauma Informed Care helpful.
Strong crisis intervention skills required.
Must have CPR Certification and first aid.
Good written and verbal communication skills.
Effective time management and organizationalskills.
Ability to work in a team setting.
Abilityto act as a positive role model.
PHYSICAL DEMANDS REGULARLYREQUIRED:
to sit, use arms/hands to reach, manipulate,handle, or feel objects, tools, or controls; written and verbal communication,ability to hear.
OCCASIONALLY REQUIRED:
to stand, walk, stoop,kneel or crouch and lift and/or move up to 10 pounds.
