The Case Manager provides case management for clients who may be diagnosed with chemical dependency or co-occurring chemical dependency and mental health disorders. The Case Manager also functions as an advocate and support person to individuals assigned to their program, links them to appropriate services, and monitors client progress.
Job Duties and Responsibilities
Advocates in the client's best interest.
Conducts client intake, screening, assessment, orientation, and related reporting.
Develops and implements case management plans addressing clients’ goals of stable housing, gainful employment, engagement in behavioral health recovery support, and linkage to ancillary services.
Creates transition and discharge plans with clients that include provisions for behavioral health disorders.
Coaches clients on engagement with treatment plans, self-help recovery services, individualized health and wellness goals, and self-responsibility.
Facilitates life skills groups following evidence‑based practices.
Prepares and maintains clinical records in accordance with policy and procedure.
Interfaces positively with community partners.
Participates in client case reviews and team meetings.
Provides referrals to appropriate resource agencies and documents outcomes of referrals.
Offers crisis intervention when needed.
Maintains client rights as defined by DCF.
Upholds the highest standards of ethical conduct and confidentiality.
Maintains clinical records in accordance with agency standards.
Required Key Performance Indicators
Complete 1352 targeted case management direct service hours per year.
All documentation completed the same day of service.
Meet recovery goals through change of level of care within set timeframe 90% of the time.
Achieve 99% accuracy in EHR information entry.
Ensure 100% of events are on the active treatment plan.
Complete 100% of required trainings within specified time frames.
No authorizations, no service.
Maintain a no‑show rate below 10%.
Required Knowledge, Skills, and Abilities
Bachelor's degree with major coursework in human services field.
Minimum of 5 years relevant experience working with adults with substance abuse/mental health issues.
Clear understanding of the characteristics and problems associated with mental health diagnoses or co‑occurring mental health and substance abuse diagnoses.
Knowledge of substance use disorders and principles of recovery.
Knowledge of Trauma‑Informed Care helpful.
Strong crisis intervention skills.
Must hold current CPR certification and first aid.
Good written and verbal communication skills.
Effective time management and organizational skills.
Computer literate.
Ability to work in a team setting.
Ability to act as a positive role model.
Physical Demands Regularly required: sit, use arms/hands to reach, manipulate, handle, or feel objects, tools, or controls; written and verbal communication, ability to hear.
Occasionally required: stand, walk, stoop, kneel or crouch, and lift and/or move up to 10 pounds.
Work Environment The work environment is that of a substance use treatment milieu, which may include exposure to clients infected with Hepatitis B, HIV, TB, or other infectious diseases.
Scheduling Needs Flexibility to work days, nights, weekends, and holidays.
