We are hiring a
Certified Patient Navigator
to support patients through their care journey. This remote role requires an active
Florida state Patient Navigator certification , hands-on navigation experience, and strong knowledge of
Medicare and Medicaid .
Responsibilities
Guide patients through appointments, referrals, and follow-up care
Assist with Medicare and Medicaid eligibility, benefits, and coverage
Provide clear and supportive patient education
Maintain accurate patient records and documentation
Communicate with clinics, care teams, and community partners
Identify barriers to care and connect patients to needed resources
Ensure full HIPAA compliance and patient confidentiality
Support patients remotely via phone, email, and telehealth
Required Qualifications
Active Florida state certification as a Patient Navigator
Minimum 1 year of direct Patient Navigator experience
Strong knowledge of Medicare and Medicaid
Experience with EHR systems and scheduling platforms
Excellent communication and organizational skills
Ability to work independently in a remote setting
Must reside in Florida and be legally authorized to work in the U.S.
