Pharmacist Per Diem
Location: Legacy Lyons – 3811 Lyons Ave., Houston, TX 77020
Schedule: Monday – Friday (7:30 AM – 8 PM) and Saturday (7:30 AM – 5:30 PM). Must be available 4 days per month, minimum of 1 shift every two weeks.

Key Responsibilities
Play detective by interpreting, clarifying, and transcribing medication orders with precision
Ensure medication is perfect, considering diagnosis, weight, and other factors
Maintain compliance with all pharmacy practice rulebooks
Profile reviews: lead drug selection and dosage decisions
Conduct reviews to keep drug interactions and duplications at bay
Document unexpected drug reactions with flair
Dispense medications and keep inventory records in tip‑top shape
Stay informed and promote evidence‑based medicine
Advise patients on health and illness prevention
Educate patients and healthcare providers with drug information
Minimum Qualifications
Graduate from an accredited pharmacy school (Pharm.D or B.S. with pharmaceutical experience)
Certification in immunization delivery and others (or on your way there)
Valid Texas Board Pharmacy License
Texas Pharmacist Preceptor license
First Aid/CPR certification
Active in Medicare, Medicaid, and other programs
2+ years of healthcare experience; post‑grad residency a bonus
Community pharmacy experience (1+ years preferred)
Medication adherence counseling experience (1+ years preferred)
Experience in HIV/AIDS, geriatrics, and/or pediatrics (preferred)
