We are looking for an experienced nurse who wants to remain clinically involved in patient care without being a bedside nurse. You should be organized, efficient, and able to manage your duties with autonomy.
Responsibilities
Work the RAI process and conduct assessments and care plan coordination for assigned residents.
Complete the MDS, CAA’s, and care plans within regulated time frames.
Assess residents through physical assessment, interview, and chart review.
Discuss resident care needs with caregivers including physician, nursing, social services, therapy, dietary, and activity staff.
Review information from hospitals, consults, and outside agencies and use it in assessment and care planning.
Coordinate, identify, and/or initiate significant changes to MDS.
Prepare for and conduct PPS meetings, maintaining MDS assessments per Medicare schedule and manage PPS board for monitoring Medicare days and RUGs utilization in the absence of the Care Management Coordinator.
Remain current with the American Association of Nursing Assessment Coordinators (AANAC) requirements.
Qualifications
Registered Nurse (RN) or Licensed Practical Nurse.
AANC certification or RAC-CT a plus.
Knowledge of the Resident Assessment Instrument (RAI) process, including PPS.
Experience as an MDS Nurse.
