The Laurels of Huber Heights is looking for an experienced MDS Coordinator to oversee the RAI process, conduct assessments, and coordinate care plans for residents. Benefits

Complete MDS, CAA, and care plans within regulated time frames Coordinate scheduling of the RAI process with the interdisciplinary team Assess residents through physical assessment, interview, and chart review Discuss resident care needs with caregivers, including physician, nursing, social services, therapy, dietary, and activity staff Review information from hospital, consults, and outside agencies to complete assessment and care planning Coordinate, identify, and/or initiate significant change in MDS Maintain MDS assessments per Medicare schedule and preserve PPS board for monitoring Medicare days and RUGs utilization in the absence of the Care Management Coordinator Remain current with American Association of Nursing Assessment Coordinators (AANAC) requirements Qualifications
Registered Nurse (RN) or Licensed Practical Nurse (LPN) licensure AANC certification a plus (RAC‑CT) Knowledge of the Resident Assessment Instrument (RAI) process, including PPS principles, strongly preferred Experience as an MDS Nurse
