Stepping Up Case Manager
– AltaPointe Health Systems Responsibilities
Coordinates and provides services related to the “Stepping Up” program (works closely with the Alabama State “Stepping Up” Coordinator, the Technical Assistance Team, and local community partners to implement processes and procedures to reduce recidivism and unnecessary re‑entry of people with mental illness or co‑occurring mental illness and substance use disorders into local county jails and hospitals) Conducts case management services for participants in the “Stepping Up” program (implements case management plan by providing case management services in conjunction with county jails and area hospital staff; advocates patient’s needs with community providers; links patients with community services; monitors patient progress) Represents the needs of the persons admitted into the “Stepping Up” program and ensures quality services are provided (participates in “Stepping Up” related trainings; completes work in accordance with AltaPointe and “Stepping Up” standards; participates in meetings of the Stepping Up Alliance Task Force) Completes needs assessment using required case management forms to determine patient assets, strengths, deficits, barriers, and needs Completes basic screenings for patient risks, substance use, functional deficits, and social determinants of health Develops case management service plan after assessing needs, consulting with referral source, and assisting patient in setting goals (which may include personal grooming, skills teaching, medication knowledge, and financial resource assistance, etc.) Implements case management plan by providing services to monitor patient condition, make home visits, assist patient in securing needed services; follows up with patient regarding referral compliance, medication compliance, and consults with treatment team when noncompliance is found; assists when patient is transitioning from hospitalization and/or jail to home or a lesser level of care to ensure needed services are obtained and beneficial Represents the needs of persons with serious mental illness and co‑occurring substance use disorder issues by establishing liaison with other community resources, identifying service gaps, and advocating for the patient; provides transportation as needed and when appropriate Participates in ensuring quality of services by attending all required case management training, completing work in accordance with standards, and participating in meetings and tasks Supervision and Consultation
Seeks supervision and consultation as needed Accepts and employs suggestions for improvement Actively works to enhance skills Clinical Record Keeping
Documents in a timely fashion per AltaPointe policy Documents in a clear and concise manner and legibly Adheres to standards in all documentation Attitudes Toward Patients and Staff
Treats patients with care, dignity, and compassion Respects patients’ privacy and confidentiality Is sensitive to patients’ needs, expectations, and individual differences Administrative and Other Duties
Actively participates in Performance Improvement activities Actively participates in AltaPointe committees as required Completes assigned tasks in a timely manner Follows AltaPointe policies and procedures Attends appropriate in‑services training and other workshops Qualifications
Bachelor’s degree in a behavioral health, mental health or social services related discipline. Valid verifiable state‑issued driver’s license. Certification of case management training by Alabama State Department of Mental Health and Mental Retardation. Knowledge Requirements (to be attained within six (6) months of employment)
Knowledge of psychiatric and substance use disorders in adult populations and/or knowledge of addiction as a primary/secondary illness Knowledge of Trauma Informed Care principles Knowledge of cultural diversity Knowledge of behavior management, crisis intervention and interventions as well as non‑violent crisis intervention practices common to acute psychotic patients, and/or patients with substance abuse issues Knowledge of educational methods appropriate for adults with severe and persistent psychiatric disorders Knowledge of rules and regulations regarding appropriate benefit entitlements such as social security, Medicaid, Medicare Knowledge of utilizing electronic health records Knowledge of available community resources Knowledge of available referral sources for physical health, mental health, social services, and substance abuse treatment Knowledge of behavioral health and substance abuse screening tools Training Requirements (to be completed within 6 months of employment)
