Case Manager w/ Rapid Access – Adult Mental Health – SIGN-ON BONUS ELIGIBLE
Richmond Behavioral Health is seeking full-time Adult Mental Health Case Managers to perform intermediate professional work assessing client needs, developing, implementing and monitoring service delivery and assistance plans, coordinating and monitoring services with other agencies, counseling and assisting clients, maintaining records and files, preparing reports, and related work as apparent or assigned.
Responsibilities
Assess client service needs, capabilities and appropriateness for services; present options and services based upon a needs assessment.
Develop an individual service plan of care and services appropriate to the evaluation with the client/family consensus; review and update service plans.
Make referrals and linkages to appropriate agencies for services; coordinate client services and treatment with multiple service providers and agencies.
Evaluate the quality of services provided and changes in client’s condition and counsel clients as necessary; evaluate client’s environment for safety, security, negative factors and productivity; compile and analyze data relating to complaints; identify and work to resolve problems.
Plan and facilitate group sessions with clients participating in community activities and/or to promote recovery and educate clients.
Assist clients with activities of daily living including hygiene/bathing.
Orient and train newly hired staff; evaluate staff performance as directed.
Serve as liaison to public agencies and provide information regarding Authority programs and services.
Attend inter-disciplinary team meetings to discuss decisions for client plan of care; provide or arrange transportation; assist with discharge planning.
Provide crisis intervention and prevention services.
Prepare a variety of reports; prepare and maintain client charts and other records.
Attend meetings, staffings and conferences as they relate to client, staff and program needs.
Attend court hearings; testify in court.
Assist in curriculum development and resource tools.
Position is full time in the building during business hours.
