Network180 is a behavioral health community organization that delivers mental health, substance abuse treatment coordination, and developmental disabilities services for Kent County. We are seeking an ACT Case Manager responsible for Person-Centered Planning to develop an Individual Plan of Service and provide ongoing linking, coordinating, and monitoring of services. The role requires 40 hours a week, Monday through Friday from 8:00 am to 5:00 pm, with some evenings, weekends, and rotating on-call shifts. Daily travel within the community is required. The position is in-person. Responsibilities

As an ACT Case Manager, you will develop and implement individualized service plans, coordinate community-based services, monitor client progress, collaborate with interdisciplinary teams, and provide crisis intervention. You will link clients to mental health and substance use treatment, behavioral health services, case management, and community resources while ensuring compliance with state regulations and agency policies. Minimum Qualifications

Bachelor’s degree in a human services related field (e.g., Social Work, Education, Psychology, Sociology) One-year full-time experience working with persons with mental illness (fieldwork and/or internship experience can apply) Valid driver’s license and reliable transportation for job-related travel Strong organizational skills with experience in person-centered planning Demonstrated understanding of persons with severe mental illness and substance use disorders Effective communication skills that convey information clearly Active listening skills (listening to others and asking appropriate questions) Time management skills Lived experiences with mental illness, developmental disabilities, or substance use disorders valued Preferred Qualifications

Licensed BSW Qualified Mental Health Professional (QMHP): one-year full-time experience working with persons with mental illness and appropriate license/degree per State of Michigan Knowledge of trauma-informed theories, principles, and practices (including community trauma, intergenerational and historical trauma, parallel processes, and universal precautions) Equal Opportunity
