Base pay range $66,199.00/yr - $79,442.00/yr

Duties & Responsibilities Purpose of Position The Addiction Counselor serves as an integral member of the multidisciplinary team and participates in and/or leads the development and implementation of clinically sound, culturally relevant, and medically monitored treatment programs, case management, care coordination and other supportive and consultative services for patients in Substance Use Disorder (SUD) treatment, in a sensitive and caring manner, and promotes the notion of recovery and self‑sufficiency, as a goal of treatment. Ensures optimal utilization of resources and services and compliance with all applicable local, state, and federal rules and regulations and NYC Health + Hospitals standards of care to improve outcomes and the patient experience.

Essential Duties And Responsibilities

Conducts initial screenings and assessments of patients, performs ongoing toxicology screenings, and develops psycho‑social histories to identify and isolate potential SUD and other specific patient problems; formulates diagnostic impressions; prepares screening, intake, and assessment reports.

Facilitates and/or co‑facilitates individual, group, and family counseling for patients; provides psycho‑education and conducts skills training classes.

Makes decisions and maintains required records regarding appropriate Level of Care using patient criteria tools, ensuring proper placement of patients.

Develops and monitors treatment/recovery plans, establishes short‑term and long‑term goals, initiates discharge planning, and documents treatment outcomes.

Participates in evaluation of treatment progress, maintains confidential case records, and records patient progress.

Participates in staff meetings, documents services provided in case records in a timely and accurate manner, and ensures compliance with reimbursement standards.

Discusses financial responsibility with patients and provides money‑management assistance.

Provides orientation to new patients, families, and significant others; establishes relationships and supports referrals, service gaps, and community resources.

Coordinates case and care management services integrating supportive services and community resources.

Provides crisis intervention and support, and seeks additional assistance when needed.

Acts as liaison, advising staff of patient needs regarding job training, housing, and education programs.

Conducts outreach and advocacy activities to educate staff and communities on substance abuse, prevention, and service availability.

Participates in internal/external QA/PI activities, programs, and trainings as required.

Holds required certifications, completes continuing education, and complies with NYC Health + Hospitals policies and applicable regulations.

Performs other related duties as directed.

Minimum Qualifications

Valid license and current registration or valid limited permit to practice as a Mental Health Counselor issued by the New York State Education Department (NYSED).
